A Medical Social Worker helps find community resources and facilitates planning with family members so the transition back home can be as smooth as possible.
While in rehabilitation, patients and families sometimes have difficulty dealing with the social, emotional and financial concerns that come with hospitalization. The Medical Social Worker is trained to help resolve those issues and ensure patients and families return to a living environment in which they can be as independent possible.
